Could be the battery as well. I had a '99 Durango that had the battery die,
completely. I started it, drove it for 15 miles, shut it off to get gas, and
then nothing. Tried a jump pack and it'd start, but not idle. I had to do
the same, drive with both feet so as not to let the RPMs drop. Later I found
out that the startup electrical load is more than the alternator can handle,
so the revs must be higher if the battery is shot. It'd settle down once
warm and running for a bit, the idle would hold, but it was shaky.
Could be a similar situation, with things being so cold. But then it could
be moisture in the fuel too...

I'm in Winnipeg, -30 C not uncommon. Nothing wrong with the truck re not
starting/flooding. At that temp the battery has only 30%of the power that
id does at room temp and the oil is quite heavey. Fuel doesn't vaporize
very well. Engine is cranking slower, ignition is weaker, fuel not igniting
as easily because it's not vaporized well, engine doesn't start. That's why
we use block heater at that temp. Probably showed an error code after
finally starting because it sensed an high HC reading via the O2 sensor and
ran ragged until it burned off all of the fuel sitting in the intake
manifold and vaporizing off the oil.
Word of caution. If you're starting at that temp with out pre-heating via a
block heater you're knocking a lot of miles off the engine's life expectancy
and running a high risk of crank failure if you're into the throttle when it
finally does start. I've seen more spun bearings as a result of this. The
oil is extremely thick and takes a while to get to everything at that temp.
In the good old days of point ignition and carberators at least they
wouldn't start. When cars started to be able to start better in extremely
cold weather we started to see more damage as I described. Used to have
their own self preservation system.
