I have NO CLUE. Sorry, is there any possibility that it was damaged during
the install? Just a thought to make sure the new one doesn't get damaged and
fail as well....

Yes, the straps will release the first time the clutch pedal is pushed.
They can even be reused sometimes.
I take it your throw out bearing and slave are the POS one unit jobs?
