Default 92 dodge VAN

I have a 92 dodge 150 conversion van with the 318.

Im having a problem with it. here in the past few days the transnission has been slipping really bad. to get it to start moving you have to put the pedal down to about 3-4k rpm and while your driving the rpm are always way too high. This thing feels almost like its in lowrange if it had one haha.
Ohh and when going up hills it tends to jerk back and forth between grabing and slipping.

Just wondering if this is deff my tranny and if so what kind of tranny it would be and where i could get the best deal on a new one and do i need to replace my torque converter also?

Is the fluid full? Have you checked it with it idling in neutral.....Not park.

If it's full it's on it's way out. You can adjust the bands and probly get some more time out of it but not much.
