I had a belt pop off once but that was on a race-prepped V-8 under full
throttle (very high RPMs). Was the belt tight? If the pulley's not broken
like Mike's asking, a loose belt (or a broken/bent bracket somewhere)
might've had enough play that the belt may have enough flex in it to ride up
and be thrown but that doesn't sound like the driving conditions you
describe. Did you put the belt back on? Has it done it since? If not, I'd
just keep an eye on it and press on with your daily driver. A new belt,
inspect the pulleys
