I also have a 95 4.0 that likes to go tickaticka every now and
then, mostly around town. Doesn't last more than half a dozen
ticks. Dunno where you'd get 92-93 octane, most premium is '91.
And the ticks in mine go away if I just move up to midgrade
rather than regular. Someday I'll try an octane additive just
for grins. Doesn't tick at all pulling any of the backroad
grades in the Sierras or high Utah desert, just down near
sealevel in the Bay area. Suspect it is the crappy gas required
by CARB. As for not burning premium due to not being as volatile,
no real relationship between octane and volatility... and if
your gas don't vaporize, your fuel injection is busted.

You have less octane at altitude because you have less oxygen and
money.

1. I't don't ticka ticka at altitude, just around sea level.

2. I'm pretty sure both Reno and Las Vegas are at lower altitude
than the Sonora or Kit Carson pass, so that won't be the cause
either.

Use a higher octane at sea level.

The higher you go the lower your compression will be and
so you can use lower octane.

I run 87 when in Colorado Springs. But have to run 93 and stay
out of max boost when down in Biloxi Mississippi.
