Hi Arron,
Yup it's much more common than the manufacturer would lead us to
believe. Keep your ears open and you'll hear many of the accidents we
see on TV and whatnot are actually cause by this C clip. Last on about a
year ago on San Diego I-5 was a full sized Suburban. killed over ten
people, counting the other cars that ran into it.

Axle retainers, like those required for drag racing would eliminate
these accidents:
